I certify that I have received the written description of the Ana Visage Academy Drug and Alcohol Abuse Prevention Student Conduct Policy. I understand that Ana Visage Academy is committed to maintaining a healthy environment in which students can learn and employees can work. I understand that there are services offered to assist students and employees in dealing with drug and alcohol addiction but that repeated violations of the stated policies could result in both terminations from enrollment/employment and referral to the local law enforcement agencies for investigation. I respect the school's position and pledge my cooperation in supporting and abiding by the standards described in the program material.
