current school catalog and tuition supplement of the AVI Career Training. I
understand that I will receive training and specified hours of instruction
as described in the catalog.

AVI Career Training reserves the right to cancel this contract and
dismiss the student from the program (s) if the student does not meet
the Standards of Satisfactory Progress, the Attendance Policy, the
Student Conduct Policy, or for non-payment of tuition or other costs.

AVI Career Training offers placement assistance but cannot in any way
guarantee employment after the student has successfully completed the
program of study.
A finance charge fee of $50 plus 15 percent per an-num is added to
unpaid tuition 30 days before completion. Student Initial

Contract is subject to all claims and defenses which the debtor
could assert against the seller of goods or services obtained
pursuant hereto or with the proceeds hereof. Recovery hereunder by
the debtor shall not exceed the amounts paid by the debtor
hereunder.
The school reserves the right to make certain changes in program
content, equipment, books, faculty, program length scheduled class
times, and tuition. Tuition and other cost changes will not affect

I agree that should my account become delinquent, I will be responsible
for all collection costs, including, but not limited to, the outstanding
balance, reasonable attorney fees, court costs, collection agency fee,
and interest at the rate of 18% per annum (1.5% per

All students are required to complete
their program by their "Enrollment Contract" scheduled completion
date. If additional time is needed due to missed clinicals or tests
of theory chapter, there will be an additional fee to extend the
graduation date. The class missed time can be made on the same week
basis with no extra charges but if the students fail to take the
initiative to make up the missed class and use the time to get up to
date with course work and test taking based on scheduled curriculum
then a fee of $28/hour or $896/week will be imposed based on 32
hours a week full time beyond their completion date. Student Initial
AVI has no prevision or means to deny or abridge the benefits if any
applicable Federal State or District law regulation designed to protect
consumer or credit purchases.
Enrollment Agreement AND catalog constitutes a binding agreement between

I ACKNOWLEDGE HAVING READ AND RECEIVED A COPY OF THIS AGREEMENT AND AGREE TO THE TERMS AND CONDITIONS CONTAINED HEREIN. I CERTIFY THAT THERE ARE NO VERBAL STATEMENTS CONTRARY TO OR IN AMENDMENT TO THIS AGREEMENT. ALSO, I HAVE RECEIVED A COPY OF THE CURRENT AVI CAREER TRAINING CATALOG
I Understand that this is
a legally binding agreement. My signature below certifies that I
have read, understood, and agreed with my rights and
responsibilities. Further, I certify that I understand the
cancellation and refund policies and I understand and agree to these
policies.
NOTE: Diploma and/or Transcript may be withheld
until student is current of all financial obligations to the school
and/or the student has completed any required paperwork.

Refunds, when due, are made without requiring a request from the
student. If a student officially withdraws from school, or if a student
is officially terminated from the School, that date becomes the Date of Determination.

In situations of mitigating circumstances, the school Director may
refund more than the amount determined by the refund policy. Granting
credit for previous training shall not impact on the refund policy.
The nonrefundable application fee,
books, supplies, and materials are for each program separately and
are not refundable. In the event of legal action to collect
any sums due here, the undersigned student shall be responsible for all
costs and attorney's fees incurred by the school and jurisdiction will

If a student officially withdraws from
school, or if a student is officially terminated from the school,
that date becomes the Date of Determination. Upon withdrawal, the
student's last day of attendance will be ascertained.

If a student does not return from a leave of absence and has not
provided a written request for an appropriate extension to that leave of
absence, the student will be withdrawn from school. The date of
determination will be the date the student was expected to return from
leave of absence. On the Date of Determination, the student's last day
of attendance will be ascertained. If any monies are due to Federal
Title IV funds, to the student, or other agency, the money will be
returned within 45 days of the student's date of Determination.

The student releases and holds harmless the institution, its employees,
its agents, and representatives from and against liabilities, damages,
and other expenses which may be imposed upon, incurred by, or asserted
against it or them by reason of bodily injury or property damage which
may be suffered by the student from any cause while enrolled as a

The student may cancel his/her enrollment agreement, by written notice,
at any time prior to the first day of the course. When cancellation is
requested under these circumstances, AVI will refund all tuition paid by
the student, less maximum tuition fee of 15% of the course or program or

By signing this contract, I affirm that the catalogue has been received,
read, understood and all my questions answered. I have read and signed
the Students orientation form and had the opportunity to ask all my
questions on Students conduct, Attendance policy, In and Out Credit
Transferability of AVI Career Training program, Satisfactory Academic
Progress Policy, Grievance Procedure, Graduation & Completion &
Licensure Requirement, Career Opportunities, Students Records Release
Policy, Instructors availability office hours by appointment, Registered
Program course content and Grand Total of all Applicable Tuition & Fees
